Forms

Forms for registration can be downloaded from this Web site. Registration forms must be returned with a parental permission form to the CSP Office in Campus Ministry (outside the cafeteria next to the counseling offices). Miss Bieber or Mr. Ehlman will gladly answer any students' or parents' questions. Missouri law requires that anyone working with young students provide written proof of a negative tuberculosis test (TB) before contact with these populations. Programs with this requirement are indicated on this website. TB tests may be obtained through your family healthcare provider or through St. Louis County Health Services for a fee. Mr. Scott Gilbert, SLUH school nurse, can read the skin test results and provide the written statement of negativity, but he cannot give the test at SLUH.

Please note that two of the venues involve feeding programs for homeless people (men, women and children). As a result, we have a food bank here at SLUH where students will be able to bring in non-perishable items throughout the school year to help decrease the costs. Even if you are unable to participate in a CSP venue, you can assist with these efforts.

PLEASE NOTE
Archdiocesan Permission Forms can only be obtained from the CSP Office.
