Freshman Service Program: 2021-2022
DATE: Fall & Spring Semesters, 2021-2022 Academic Year, after school hours
I. Description of the Field Trip Activity
In the Freshman Service Program, each freshman participates in two after-school service opportunities, one each semester. Both these opportunities begin immediately after school and introduce students to two types of service offered through SLUH.
One semester, each freshman will help make a meal at school to be served at Garfield Place, a division of Peter & Paul Community Services that provides permanent, supportive housing for people who have experienced chronic homelessness. Ingredients used for these meals are mostly the fruits of the school's annual Food Drive. Students will usually be ready to leave SLUH by 5:15pm.
The other semester, each freshman will spend an afternoon visiting and playing games with older adults at McCormack House, a senior living community within walking distance of SLUH. Freshman will be back at SLUH and ready to be picked up by 5:20 p.m.
Students will be assigned to both of his service dates, along with five to seven other freshmen and a faculty leader. A faculty moderator will either use a SLUH van to drive the group to Garfield Place or will accompany the group on the short walk to McCormack House.
II. Objectives of the Field Trip
A key component of SLUH’s mission as a Catholic, Jesuit school is to form young men dedicated to service with the poor and marginalized. It is our hope that after an introduction to SLUH’s service opportunities, freshman will continue to serve the St. Louis community and beyond through SLUH’s voluntary Community Service Program (CSP).
III. Provide Specific Unique Materials, Equipment to be Brought
The school covers the entire financial cost of each student’s service experience.
Parents may use this area to write in specific instructions for their son and/or list any medical concerns the adult leaders should be aware of.
By signing this Form, I (the parent or guardian) certify that I request and give my permission for this student to go on this field trip.
I am aware of the requirements of the field trip and have talked these over with my son. I release the teacher, principal, and school from all liability and waive and claims against them.
Only students with this signed permission form will be permitted to go on the field trip. This permission slip is due to Campus Ministry by Tuesday, May 4.

Please list any allergies/pre-existing conditions that you want emergency personnel to be aware of.
In case emergency treatment is needed for your son, he will be taken to the hospital nearest to where the injury takes place.
I hereby give permission for a physician to administer emergency treatment to the above named student.
