SLUH Mixer rules:

Mixer exclusively for students enrolled in the 9th grade.
Doors open at 7:30 p.m. Attendees must arrive by 8:00 p.m.
Students cannot leave until 9:30 p.m., and must be picked up right away at 9:30 p.m.
NO: backpacks, bags, whistles, noisemakers, glitter, or vaping.
Attendees will be breathalyzed as they enter.
This form must be filled out to enter.
Form confirmation email is required to enter campus and mixer. Name on email must match your school ID.
Casual school appropriate dress is required.

I understand that I, along with my student, agree to follow all rules laid forth by SLUH. Should my student not follow the directives put forth by SLUH for this event, I am aware that she will be asked to leave the event. I also recognize that there are inherent risks associated with my student's attendance at this event, and thus, hold SLUH and people within its organization harmless of any legal action as a result of this event.
